Requirements to Participate in Fall Athletics and Activities

High School Fall Athletics & Activities

We are getting ready to start practices at Brookings-Harbor High School. The first day of official practice is Monday, August 19, for High School Fall Season. Fall Athletics include Soccer, Football, Volleyball, Cross Country, and Cheer. Fall Activities include Marching Band and Color Guard. Visit the BHHS Athletics & Activities page.

To participate in fall athletics and activities, you will need:

  • Documentation of a current physical by a medical care provider no more than 2 years old.

  • Online activity registration completed via Arbiter Sports: https://students.arbitersports.com/organizations/brookings-harbor-school-district

  • Pay the $85.00 Sports Fee - if you need assistance paying your fee, please see Tina Peters in the high school office.

Once this is done, students receive a “Gold Card” that can be presented to their coach so they know that the student is cleared to participate.

Middle School Sports

Middle school fall sports practices will start after school begins, unless you are notified by your coach of a different date. Azalea’s fall sports seasons include Cross Country, Football, Volleyball, and Cheer.

Azalea Middle School student-athletes must register via Arbiter Sports, pay a $30-$60 fee, and have a current sports physical form completed. You can find information on middle school sports posted here or call Corrie Taylor at the middle school office at 541-469-7427 Option 3.

Sports Physicals

Student-athletes are required to have a sports physical on file every two years. To learn more about requirements for participation in school athletics and activities or to download the new 2024-2025 form to take to your own medical provider, see the BHHS Athletics & Activities webpage.

Coast Community Health will be on site for a Sports Physical Clinic - offering low cost exams for students who need an updated exam to participate in Fall 2024 athletics and activities.

Date:  Thursday Aug 15th  8:40 am - 4pm Appointments are Required.  Please call 541-332-1114 (Coast Community Health Center, Port Orford Office) for an appointment.  Please make sure to let the front desk know you are scheduling for the Brookings Sports Physical Day.
Cost: $40  Cash or Check only.  Checks Payable to Coast Community Health Center. A Sliding Fee Scale is available with an application and proof of income, completed at time of the appointment. 
Location: Bridges Building, 564 Fern Ave., Brookings (adjacent to the football field parking lot). 
Information: Paperwork Must be Completed in Advance and brought to the appointment.  It can be found on the website here:  https://www.coastcommunityhealth.org/patientforms/ or can be picked up at AMS or BHHS main offices between the hours of 8 am and 4 p.m. beginning on Aug. 5, 2024. 

Please note this will be the only day sports physicals will be offered through the school district School Based Health Center this year.  A limited number of appointments are available, call to schedule ASAP. 

If your student needs a physical and you can not get an appointment at this clinic, please schedule with your child’s medical provider ASAP to obtain a sports physical to prevent missing out on practices and competitions.
